About the position:

The Stellar Care Coordination Team works closely with providers and insurance companies to help monitor and improve patient quality of care. This is done through various preventive care coordination initiatives.

The Care Coordinator conducts telephonic outreach to patients to encourage compliance with primary and preventive health maintenance visits and to educate members on how to access services within the assigned area. Coordinator(s) in this position will work as an extension of medical practices outreaching patients.

The Coordinator in this role support providers and Stellar Operations team with:

  • Coordinating and monitoring member appointments for preventative health visits via the web-based Stellar application. Training will be provided on the Stellar application.
  • Serving as an extension of Stellar medical practices to help communicate health needs and ensure patient/patient families complete important appointments
  • Care Coordinator(s) must be comfortable communicating telephonically with patients, providers, and pharmacies.

As a Care Coordinator at Stellar, you will work closely with internal stakeholders including practice managers and operational leads working collaboratively to meet company goals.This role will report into Stellar’s Chief Medical Officer. This position primarily works remotely conducting telephonic outreach but may require some contact with provider offices.

As a Senior Outreach Coordinator, you should have:

  • High school diploma or GED; Associate Degree or higher preferred
  • Previous health care coordination experience or customer service experience in a health-related field or environment (doctor’s office, hospital or health clinic, or member service representative of a health plan).
  • Strong communication and interpersonal skills; excellent oral and written communication skills with a knack for building relationships with patients over the phone.
  • Dedication to delivering world-class customer experience. You exhibit empathy, courtesy, helpfulness and efficiency.
  • Strong critical thinking, organizational skills and the ability to work effectively in a fast paced, ever evolving environment
  • Knowledge of clinical terminology and health information systems strongly preferred; familiarity with reviewing medical charts.
  • Experience utilizing electronic medical records and/or online scheduling systems
  • Proficiency in MS office (including google spreadsheets). Ability to learn Stellar documentation and care coordination applications.
  • Bilingual (English/spanish) not required but a plus
  • Ability to work Monday - Friday 8 hours in your time zone of residence.

Pay:

At Stellar, we believe in transparency and we do our best to make sure the company and our candidates are on the same page as it relates to compensation. In addition to posting salary ranges for our open roles, candidates should expect to be asked about compensation expectations and requirements early on in their interview process. Our goal is to highlight when expectations and Stellar's salary range may be out of sync, and work with the candidate to determine whether it makes sense to continue conversations.

Where a new hire falls within this range will be based on their individual skills and experience, and how these competencies compare across other employees in the same role. Stellar's bands are designed to allow for individual compensation growth within the role. As such, new hires typically start at the lower end of the range. Stellar rewards performance and outcomes - should you join the company, you will have the opportunity to grow your salary over time.

  • The base salary range for a Senior Outreach Coordinator is $70,000-$90,000 and will be eligible for equity and an annual performance based bonus.
