About the Position:

Stellar Health is looking for a Lead, Total Cost of Care to join our Team. This person will work closely with Stellar’s SVP and VP of Total Cost of Care and overall Operations, Analytics, Product and Finance teams.

This position will be responsible for the day-to-day operations of Stellar Health programs focused on improving Population Health Outcomes through reducing total cost of care and driving performance in value-based risk contracts, as well as supporting the contracting process for entering into new value-based contracts and models. This position will be tasked with implementing and improving operational strategies to optimize care coordination, fostering collaboration and communication among key healthcare stakeholders, achieving financial and quality targets set by CMS and payors, all facilitated by the Stellar Health Platform. This role requires an understanding of value-based care models and healthcare operations and economics.

What you'll do:

  • Help develop and manage the strategy, delivery, and execution of organization's MSSP ACO and other value-based programs
  • Work collaboratively with leaders across Stellar Health to achieve strategic goals related to the development and implementation of the total cost of care strategy and programs.
  • Develop strong working relationships with participating practices, facilitated by Stellar’s provider operations team, in order to drive key messages, actions and participation in any provider forums
  • Provide operational support for ACO program activities to ensure compliance with CMS, payor policy and other regulatory requirements.
  • Develop business and product requirements to support total cost of care management activities.
  • Act as centralized internal point of contact to ingest and prioritize various requests related to Stellar's MSSP ACO program
  • Help manage timely completion of all ACO-related deliverables and reporting requirements
  • Act as a key point of contact for vendors, payers, and providers in a partnership development capacity
  • Be responsible for developing materials (powerpoints, excel analyses) for internal and external executive communication regarding the health of Stellar's MSSP ACO and other value-based contracts
  • Support development of all internal and external communication related to the ACO
  • Collaborate with Finance and Operations to forecast financial outcomes of at-risk programs

As Lead, Total Cost of Care, you should have:

  • 5+ years of experience in the healthcare industry
  • 3+ years of experience with of value-based payment models, preferably MSSP ACO and other CMS/CMMI programs
  • Stays up to date with latest CMS models and industry trends
  • Has experience in value-based care contracting and/or operational performance and understands key financial levers that drive success
  • Possesses a strong consulting toolkit including data-analysis, presentation building, project management, and communication skills
  • Self-starter who is comfortable in a fast-paced start-up environment
  • Bachelor’s degree in a relevant field

Pay:

At Stellar, we believe in transparency and we do our best to make sure the company and our candidates are on the same page as it relates to compensation. In addition to posting salary ranges for our open roles, candidates should expect to be asked about compensation expectations and requirements early on in their interview process. Our goal is to highlight when expectations and Stellar's salary range may be out of sync, and work with the candidate to determine whether it makes sense to continue conversations.

The base salary range for the Lead, Total Cost of Care is $120,000 - $140,000 and will be eligible for equity and an annual performance based bonus.

Where a new hire falls within this range will be based on their individual skills and experience, and how these competencies compare across other employees in the same role. Stellar's bands are designed to allow for individual compensation growth within the role. As such, new hires typically start at the lower end of the range.
